You misunderstood the entire point. The spike and head bash already seen in the first level automatically give the new game an 18+ rating, PEGI would not give those scenes a 16+ rating. Crystal Dynamics have not been able to put any realistic death animations in since they wanted to keep the 16+ rating in the past. For the reboot they decided they would rather make a decent game than stick with the kiddie safe rating. It will be rated 18+, Crystal Dynamics are not going to change the game now to pander to some whining underage group of gamers. Crystal Dynamics guessed wrong with Legend, since the ratings got stricter. They do not need to guess to know what they could not include in a 16+ rated game is automatically going to make the reboot +18.

They would not do a new render so similar to the first one, it is Lara's head, we already had that. It might be the head of the improved ingame Lara, might be something to do with the voice actress or might be just something for a competition. I doubt it is a new render. The shape of the object it is on seems strange though, it is not straight on one side. Maybe it is the prize for the next competition.

Actually, I will take a guess it is a boxed Lara head, a bust of Lara.
