The Crossroads promo pic could well be the official cover. It was used on the flags outside E3 shown on TR's Facebook page. Actually, they could be saving the best for last, the official cover art would probably be released somewhere in a few months from now...

Anyway, I speculate Sam being killed off as an attribute to Lara turning into the badass Lara people know her as.
