Considering it's fairly near the start of the game the sanatorium is certainly a big area, and pretty tough too during the first playthrough. I got lost a good few times while I was still getting my head around using the Otherworld as a means to access further areas in the normal world.

Aparently it was heavily inspired by The Danvers State Mental Hospital (where Session 9 was filmed).



You can see the similarities on the maps too.

Well they outright said that Cedar Grove Sanatorium is their homage to Danvers (whis was torn down unfortunately) and they kept the spider like structure of the layout .
